Functionality:

* add list mode to allow nodes to be inserted in any arbitrary location
* add "*_hash_iterate()" function
* add flags argument to *_list_del() that allows the listptr to be set
to the previous or next element
* add a generic pointer type to replace *_listptr_t and *_hashptr_t ???
