- Use a single cached page map.
- Fix calculation of ending page.

This is an OpenGL renderer-specific data type. Given that, this type
shouldn't be used within the base interface for the rasterizer. Instead,
we can pass this information to the rasterizer via reference.

This makes clang-format useful on those.
Also add a bunch of forgotten transitive includes, which otherwise
prevented compilation.

This removes explicit checks sprinkled all over the codebase to instead
just have the SW rasterizer expose an implementation with no-ops for
most operations.

- Also renames "vertex_shader.*" to "shader_interpreter.*"
